Primary dysmenorrhea is the medical name for the recurring, severe period cramps (period pain) that are not caused by any disease of the reproductive organs or a physical problem of the reproductive organs. It is the most common form of menstrual pain, with 80% of girls in India suffering from it, and usually starting within a year or two after a girl gets her first period.
What Causes Primary Dysmenorrhea?
Primary dysmenorrhea is driven by natural chemical activity inside the uterus:
- Production of Prostaglandins: Before your period starts, the lining of the uterus produces chemical messengers called prostaglandins. You can think of them as the body’s natural pain chemicals.
- Uterine Contractions: Prostaglandins signal the uterus to contract and shed its lining. Higher levels of prostaglandins lead to stronger, more violent squeezing of the uterine muscles. Because of this squeezing action, the pain comes in waves – meaning it intensifies when the uterus contracts, subsides as it relaxes, and then intensifies again.
- Temporary Oxygen Restriction: This intense squeezing temporarily compresses nearby blood vessels, cutting off the oxygen supply to the uterine muscle. It is this temporary lack of oxygen that triggers severe, sharp pain signals.
Key Symptoms of Primary Dysmenorrhea
- Throbbing or cramping pain in the lower abdomen (just below your belly button), starting 1–2 days before or right as bleeding begins.
- Pain that radiates to the lower back and inner thighs.
- Accompanying symptoms like nausea, fatigue, headache, or digestive upset.
- Pain that typically peaks during the first 24 to 48 hours of bleeding.
